In 2026, FHIR matters for three specific reasons: the CMS Interoperability and Prior Authorization Final Rule (CMS-0057-F) requires FHIR-based prior authorisation APIs from most payers by January 1, 2027; the HRSA UDS+ framework requires FHIR Bulk Data submissions from FQHCs beginning with 2023 data; and quality measure reporting across Medicaid, Medicare Advantage, and CCBHC programmes increasingly relies on structured, interoperable data. Organisations whose EHR platforms are not FHIR-ready face both compliance risk and ongoing administrative cost burdens that FHIR-enabled workflows eliminate.

CMS-0057-F, published January 17, 2024, requires Medicare Advantage plans, Medicaid managed care plans, CHIP managed care entities, and QHP issuers to implement FHIR-based APIs for prior authorisation, patient data access, and payer-to-payer data exchange. Because these payer types fund services across virtually all healthcare specialties, this rule affects the payers most provider organisations work with. Most prior authorisation process requirements took effect January 1, 2026. API development requirements apply from January 1, 2027.

A genuinely FHIR-ready EHR should be able to generate patient-level FHIR Bulk Data exports in the format required for federal reporting submissions such as UDS+; support SMART on FHIR app authorisation for third-party integrations; connect to TEFCA-aligned Qualified Health Information Networks for nationwide data exchange; and produce FHIR-based prior authorisation requests compatible with payer APIs under CMS-0057-F. TEFCA (Trusted Exchange Framework and Common Agreement) is a nationwide framework established by ONC to enable secure health information exchange across different networks. ONC’s January 2025 final rule codified TEFCA provisions into federal regulation. Qualified Health Information Networks — QHINs — operate under TEFCA to provide a common network layer for data exchange. For provider organisations, TEFCA enables the kind of cross-organisation data sharing — referral coordination, discharge follow-up, care transitions — that currently relies on phone calls and faxes.
